How do I get someone enrolled?
Who can you hire?
- CDPAP allows you to choose who to hire for your care.
- You can hire friends and eligible family members.
- You cannot hire your spouse or designated representative.
- All employees must be 18 years of age or older.
- Your employee cannot reside at your residence unless your level of care requires such.

Who can you hire?
Prior to beginning work your Personal Assistant must submit the following:
- Completed Application
- Completed Tax Forms
- Two Original Forms of Identification
4. Physical (required annually)
5. Proof of Rubella and Rubeola Immunization
6. PPD (required annually)
