What is CDPAP
Consumer Directed Personal Assistance Program, CDPAP, is a Medicaid program provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living or skilled nursing services.
Services can include any of the services provided by a personal care aide, home health aide or nurse.
Consumers have flexibility and freedom in choosing their caregivers.
The consumer or designated representative acting on behalf of the consumer (such as parent of a disabled or chronically ill child) assumes full responsibility for hiring, training, supervising, and terminating the employment of the caregivers.
What Are The Requirements of CDPAP?
Consumer or consumers designated representative must be able and willing to make informed choices regarding the management of the services received. They must also be responsible for recruiting, hiring, training, supervising and terminating caregivers; arrange for back-up coverage when necessary; and approve payroll time keeping record.
How Do I Get CDPAP Services?
Your doctor will need send a completed Physician’s Order for Services to your local social services district office. A nursing assessment will be scheduled to determine whether you can appropriately participate in CDPAP, and recommend the amount, frequency and duration of services.
Who can be hired as a personal assistant?
- CDPAP allows you to choose who to hire for your care.
- You can hire friends and eligible family members.
- You cannot hire your spouse or designated representative.
- All employees must be 18 years of age or older.

Personal Assistant Requirements
Prior to beginning work your Personal Assistant must submit the following:
- Completed Application
- Completed Tax Forms
- Two Original Forms of Identification
4. Physical (required annually)
5. Proof of Rubella and Rubeola Immunization
6. PPD (required annually)
